Form channel
You can create a survey to collect information in a set format. It supports various methods such as text input, multiple choice, and checkbox, and you can apply styles such as themes.
Create a channel
Type /Survey in the blank space . Or click + on the left side of the block and select Survey .
Write a channel name and description
The channel name and description are stored in the preview information that you see when you share a link.
Select survey type
After filling in the blank space in the survey channel, select the survey type. Short text, long text, email, phone number, number, checkbox, drop-down, and file attachment methods are supported.
How to set options by survey type
Hover your mouse over a survey item and select ... that appears on the right.
Support options e.g. placeholder text, attachment size, multiple selection, required question, etc.

Page break
Select / Enter where you want to divide the page and then select Split Page . You can see the automatically generated Next button and page divider line.
Respondents can check the results by selecting the following buttons to see the divided pages.
Apply styles, add content
Just like the page, you can select Custom at the top to apply width, cover, theme, etc., or enter / in the blank space to add various images, tables, videos, etc.
Edit Completion Guide Page
Selecting the Survey Results button at the top or selecting Modify Completion Page at the bottom will take you to the Completion Guide page.

Share
Select the Share icon in the upper right corner. Select Copy Link to copy the survey page link to your clipboard.
In view mode, you can check the number of visitors and inflow paths by selecting the analytics icon in the upper right corner.
Selecting the QR Code button will download the QR code that will allow you to access this page as an image (SVG and PNG formats supported).
Setting the scope of survey participants
Select Member⋁ in the upper right corner and click on the writing permission. If you set it to Anyone , all visitors can respond to the survey without a separate process.
Check the survey results
The Summary tab provides a dashboard where you can view your survey visit, submission, and bounce metrics.
You can check the submitted and unsubmitted responses separately in the Results tab.
Check your incomplete responses to see which questions have low response rates and revise your questions to increase response rates.
Set Survey Options
In the Settings tab, you can set the survey progress/end settings, end time, etc.
When a survey is submitted, a push notification will be sent to the survey channel owner and administrator. Download the SlashPage app !
