Form channel

You can create a survey that collects information of a certain type. A variety of methods are supported, such as text input, multiple choice, and check boxes, and styles such as themes can be applied.
Create a channel
Type /form in the empty space . Or click + on the left side of the block and select Form .
Name the channel and write a description
The channel name and description are reflected in the preview that appears when you share a link.
Select question type
Enter / in the blank space of the form channel and select the question type. Short text, long text, email, phone number, numbered choice, check box, drop down, and file attachment are supported.

How to set options for each question type
Hover the mouse over the question and click ... that appears on the right.
Supported options: placeholder text, attachment file size, multiple selection, required questions, etc.
Page break
Enter / at the position where you want to divide the pages and select Page Break . You can see the automatically created next button and page break line.
Respondents can move on to the next page by clicking the Next button.
Customize and add content
Just like a page, you can select Custom at the top to apply form width, cover, theme, etc. Type / in the empty space to add images, tables, videos, and more.
Edit completion page
Select Results button at the top or Edit completion page at the bottom to go to the completion page.

Share
Select the share icon at the top right. Select Copy Link to copy the form link to your clipboard.

If you want to shorten the link, try applying a clean URL .
If you click Analytics Overview icon at the top right in view mode, you can check the number of visitors and funnels.
If you click the QR code button, you can download the QR code to access this page through the code. (SVG and PNG formats supported)
Set who can respond
Click Members⋁ in the upper right corner and click Post permission. If you set it to Anyone , all visitors can respond to the form without any additional steps.
Check survey results
In the Summary tab, you can check the visit, submission, and drop-off metrics of your form in the dashboard.
In the Results tab, you can view completed and uncompleted responses.

Check for uncompleted responses to identify questions with low response rates and edit them to increase response rates.
Set form options
In the Settings tab, you can set form progress/close settings, schedule closing time, etc.

When a response is submitted, a push notification is sent to the owner and manager of the form channel. Download the SlashPage app to get real-time notifications!
